Multiple reviews have established that mental health issues — postnatal depression, anxiety, psychosis, perinatal trauma — affect 15-20% of mothers, with consequences for both mothers and children. Investment is improving but remains inadequate.
What's been implemented
Mental health screening at 6-8 week postnatal check (now standard). Specialist perinatal mental health teams in each ICB (England). Maternal mental health services extended to cover first 2 years (rather than first 6 weeks). Some increase in perinatal psychiatry availability.
Where gaps remain
Waiting lists for specialist services often 3-6 months. Major regional variation in service availability. Black and Asian mothers significantly underdiagnosed and undertreated. Severe cases (psychosis, suicidal ideation) sometimes don't reach inpatient mother-and-baby units before crisis.
What to do if affected
GP first (familiar with primary care pathways). Be specific about symptoms. Don't accept dismissal as 'baby blues' beyond 2-3 weeks postnatal. Online services (PANDAS, NCT) offer support while NHS waits run. Private psychiatry available if needed (£250-400 per session).
